17-HYDROXYPROGESTERONE (17-OHP)
This test measures the level of 17 hydroxyprogesterone in the blood. It is a steroid hormone produced by the adrenal glands and gonads. The test is mainly used to evaluate adrenal gland function and to help diagnose congenital adrenal hyperplasia. It is commonly ordered in newborns, children, and adults when there are symptoms related to hormone imbalance. In adults, this test is useful in assessing causes of irregular menstrual cycles, infertility, excess facial or body hair, and unexplained acne. In newborns, it plays an important role in early detection of inherited adrenal disorders.

17 hydroxyprogesterone levels are typically elevated in patients with congenital adrenal hyperplasia, especially due to 21 hydroxylase deficiency. Mild to moderate elevation may also be seen in adrenal tumors or stress related conditions. In females, elevated values can be associated with polycystic ovary syndrome or other causes of hyperandrogenism. In males, interpretation should be correlated with clinical findings and other adrenal hormone levels. Sample timing is important. For adults, early morning collection is preferred due to diurnal variation. In women, the phase of the menstrual cycle should be considered while interpreting results. Correlation with cortisol, ACTH, and androgen levels is recommended for accurate diagnosis.
