Membership Coverage:
This card is valid for up to 7 family members as registered
at the time of enrolment. Member names cannot be changed or transferred once
the card is issued.

Validity:
The membership is valid for 12 months from the date of
activation. Renewal charges apply after expiry.
3. Discount Applicability:
Discounts are applicable only at M Star
Diagnostics and its authorized branches. Offers are not
transferable or valid with any other promotions or packages. Discounts apply only on
in-house services (not on outsourced tests or third-party
procedures).

Identification:
Members must present their valid Membership Card or
registered mobile number before availing discounts.
5.
Home Collection Service:
Free home sample collection is available within 3 km from the
center. Additional distance may attract extra charges.
6. Pharmacy Discounts:
18% discount applies only to medicines purchased
at partner pharmacies. Discounts do not apply to controlled substances,
special-order drugs, or already discounted items.
7.
Diagnostic & Imaging Services:
Discounts apply to listed investigations — Lab Tests, ECG, X-Ray, Ultrasound,
CT, and MRI — as per prevailing rate cards.
8.
Non-Refundable:
Membership fees are non-refundable and non-transferable
under any circumstances.
9.
Misuse of Card:
Any misuse, fraud, or tampering with the membership will lead to immediate
cancellation without refund.
10. Company Rights:
M Star Diagnostics reserves the right to modify, suspend, or withdraw
any part of the membership benefits, discounts, or terms without prior notice.
11. Dispute Resolution:
In case of any disputes, the decision of M Star Diagnostics Management
shall be final. Jurisdiction will lie with local courts only.
