Basic Smart Health Card – Terms & Conditions

1. Eligibility

·         This card is non-transferable and can be used only by the registered cardholder.

2. Validity

·         The card is valid for 3 Months from the date of issue, unless otherwise stated.

3. Benefits

·         Applicable discounts and offers under the Smart Health Card program are subject to change without prior notice.

·         Benefits are valid only at M Star Diagnostics branches.

4. Usage

·         The card must be presented at the time of billing to avail of discounts.

·         Discounts cannot be combined with other offers or promotions unless specified.

5. Exclusions

·         The card does not cover emergency services, home collection charges (unless stated), or outsourced tests.

6. Payments & Refunds

·         The card fee is non-refundable and non-transferable under any circumstances.

7. Loss or Damage

·         In case of card loss or damage, a duplicate card may be issued at an additional cost.

8. Right to Amend

·         M Star Diagnostics reserves the right to modify, add, or remove any terms, conditions, and benefits without prior notice.

9. Dispute Resolution

·         All disputes are subject to the jurisdiction of the local courts where M Star Diagnostics is located.
