1. Eligibility
This health card covers up to five (5) family members including
spouse, children, or parents. The card is non-transferable and valid only for
the registered individuals. M STAR DIAGNOSTICS reserves the right to verify the
identity of the cardholder before extending any benefit.
2. Validity
& Auto-Renewal
The card remains valid for 12 months from the date of activation.
To ensure uninterrupted benefits, the card will be automatically renewed every
12 months. Renewal fees will be charged to the registered payment method.
To opt out of auto-renewal, a written request must be submitted to
M STAR DIAGNOSTICS at least seven (7) days before the renewal date. Once
processed, the renewal fee is non-refundable under any circumstances.
3. Benefits
All discounts under this program are flat and fixed as per the
current plan.
Benefits are exclusively available only
at M STAR DIAGNOSTICS-owned branches and
are not valid at any partner, franchise, or third-party diagnostic centers.
The company reserves the right to revise or withdraw benefits
without prior notice.
4. Usage
The health card must be presented at the time of billing to avail
of discounts.
Discounts cannot be combined with any other promotions, packages,
or offers.
Benefits are applicable only for services provided directly by M
STAR DIAGNOSTICS.
5. Exclusions
The health card does not cover emergency medical services,
outsourced tests, hospital charges, or corporate packages.
Home collection services are available on request with nominal
charges.
6. Payments
& Refunds
The card purchase fee and renewal fee are strictly non-refundable
and non-transferable.
Misuse or fraudulent use of the card will result in immediate
cancellation without refund.
7. Loss
or Damage
In case of loss or damage, a duplicate card can be issued upon
written request and payment of applicable replacement charges. The original
card will be deactivated once a duplicate is issued.
8. Right
to Modify
M STAR DIAGNOSTICS reserves the right to modify, amend, or
withdraw any benefit, term, or condition at its sole discretion without prior
notice. Continued use of the card implies acceptance of any updated terms.
9. Limitation
of Liability
M STAR DIAGNOSTICS shall not be responsible for any indirect, incidental,
or consequential loss arising from the use or misuse of this card. Benefits are
provided on a best-effort basis at M STAR DIAGNOSTICS centres.
10.
Dispute
Resolution
any disputes arising from the use of this card shall be subject to
the exclusive jurisdiction of the competent courts where M STAR DIAGNOSTICS’
registered office is located.
The decision of the management shall be final and binding in all
matters.
